Agreement Summary:

By purchasing, “I” the purchaser understand:

• This is not medical advice
• This is not therapy
• I assume full responsibility for any and all damages, which may incur through participation. 

• If paying monthly, I assume full responsibility for paying the full amount of the program.
• This program is non-refundable.

Agreement Details:

NOT THERAPY: I understand that Nicole Mantilla is offering me a coaching program and it is not, nor does it replace, therapy, counseling or psychoanalysis.

WAIVER: I understand that my image may be edited, copied or used for educational or marketing purposes and waive any and all rights to royalties or compensations related to using my voice, image or video.

PAYMENTS: I agree to pay the full amount according to the payment plan chosen. The program fee is non-refundable, and no refunds or credits will be issued for any reason, including, but not limited to, failure to complete the program, dissatisfaction with the program, or any other reason.

LATE PAYMENTS: I understand that if I am on a payment plan and should I fail to remit any of the required payments on time, I will receive an email or other written notice from the office of Nicole Mantilla. If, after 7 days, I still have not made the late payments, may terminate further program services until I am caught up with payments. I will be responsible for the remaining balance owed to Nicole Mantilla, regardless of any termination of services attributable to late payments or other cause.

By booking: I understand that energy work consists of techniques used for stress reduction and relaxation. I understand that energy work practitioners neither diagnose conditions, prescribe or perform medical treatment, prescribe substances, or interfere with the treatment plan of a licensed medical professional. I understand that energy work does not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that energy work may complement, but should not be used as a substitute for, any medical or psychological care I may be receiving. I understand that a hands-on or hands-off energy work technique is my personal choice. I acknowledge that long term energy imbalances in the body sometimes require multiple sessions. I understand that individual results may vary by client; therefore their effectiveness depends on the client and no guarantee can be made regarding the results of its use. I understand and agree that Nicole Mantilla claims no responsibility whatsoever to any person or entity for any liability, loss, or damage caused, or alleged to be caused, both directly and indirectly, as a result of the session I received today. I also agree to contact Nicole Mantilla in the event of any issues that should arise from the session I received today. I understand that all information shared with Nicole Mantilla will be held in the strictest of confidence.

Readings and guidance is subject to my own interpretation. I agree that any information or guidance provided does not constitute or substitute for legal, psychological, financial, medical, or business advice.

I understand and agree that Nicole Mantilla claims no responsibility whatsoever to any person or entity for any liability, loss, or damage caused, or alleged to be caused, both directly and indirectly, as a result of the the session I received today.

I understand that all information shared with Nicole Mantilla will be held in the strictest of confidence. 

Yoga & Movement Waiver & Release Form:

I understand that yoga includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from the instructor. I will continue to breathe smoothly. I assume full responsibility for any and all damages, which may incur through participation. 



Yoga and intuitive movement is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By signing, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program. In addition, I will make the instructor aware of any medical conditions or physical limitations before class.

If I am pregnant, become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against Nicole Mantilla RYT Yoga Instructor.

 

AGREEMENT SUMMARY:

I have read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in the State of New York.
